  1. Proof of AADHAAR is not provided for the AADHAAR mentioned in the application
    • We request you to kindly submit a copy of AADHAAR allotment letter issued to you by UIDAI.

  2. Proof of Identity of Representative Assessee has not been submitted with application.
    • Submit proof of identity where name in application exactly matches with name in proof.

  3. Application form cannot be signed by Minor.
    • Submit specimen signature of the Representative Assessee.

  4. Signature of Representative Assessee is required.
    • Submit specimen signature of Representative Assessee.

  5. No proof for date of birth provided with the application.
    • Submit Date of birth proof of applicant where name and date of birth in application matches exactly with corresponding details in date of birth proof.

  6. Proof of address of Representative Assessee is not provided with the application.
    • Kindly submit proof of address of Representative Assessee where name and address in the proof exactly matches with details in application.
